Nursing care of a pregnant woman with primary mediastinal large B-cell lymphoma complicated with airway stenosis

To summarize the nursing experience of a pregnant woman with primary mediastinal large B-cell lymphoma complicated with atmospheric tract stenosis. Nursing key points include inventory management to ensure the safety of ECMO two-way transport;setting up an exclusive nursing team to ensure the continuity of nursing process and effective implementation of decision-making;staged coagulation monitoring and management during ECMO and CRRT operation;doctor-nurse cooperation was carried out according to the patient’s condition to predict the related complications and formulate the process plan,to provide a safe guarantee for the prevention of airway obstruction,tumor lysis syndrome,catheter-related bloodstream infection and other related complications. At the same time,patients are provided with refined pregnancy management and individualized psychological intervention. The patient was hospitalized for 9 days. After careful treatment and nursing,the patient’s condition was and improved transferred to the department of obstetrics for further treatment.

Investigation and analysis of influencing factors related to financial burden among pulmonary artery hypertension patients

Objective To investigate subjective and objective financial burden and its correlation with health-related quality of life(HRQOL) among pulmonary artery hypertension(PAH) patients. Methods From February to September 2022,we conducted convenience sampling method to select PAH patients who were followed up in a tertiary hospital,using General Information Questionnaire,Comprehensive Score for Financial Toxicity Functional Assessment of Chronic Illness Therapy(COST),and PAH-specific HRQOL measurement(EmpPHasis-10). Logistic regression model was used to analyze the independent influencing factors. Results 152 PAH patients were included,and 123(80.9%) patients experienced financial burden,and the mean score of subjective financial burden was 16.22±10.19. The multivariate linear regression analysis showed that pulmonary artery systolic pressure(PASP),6 minute walk distance(6MWD),social-economic status,catastrophic health care expenditures,and cost-related medication nonadherence(CRNA) were influencing factors of financial burden,which accounted 42.9% of the total variation. Financial burden was correlated with EmpPHasis-10(P<0.05). Conclusion The subjective and objective financial burden of PAH patients were common,and the HRQOL of patients with high financial burden level was seriously impaired. Medical staff should pay attention to and comprehensively screen the financial burden to prevent patients from becoming poor due to disease in the stage of disease management,so as to reduce the impact of financial burden on patients’ prognosis.

The nursing experience of a patient with nephrotic syndrome complicated with pneumocystis and cytomegalovirus severe pneumonia was summarized. Nursing essentials include:nursing care of high flow vein - extracorporeal membrane oxygenation therapy with continuous renal replacement therapy of pressure monitoring;pipeline safety nursing of patients with the waking extracorporeal membrane oxygenation treatment,nursing care of continuous delirium condition;early recognition and treatment of blood coagulation dysfunction in the digestive tract hemorrhage,deep vein thrombosis prevention and nursing care;infection prevention and control of immunosuppressive state,nursing care of early lung rehabilitation and psychological intervention of patients. The patient was hospitalized for 51 days. After careful treatment and nursing,the patient gets better and was discharged.

The nursing experience of a patient with severe respiratory failure caused by 1,3-hexafluorobutadiene poisoning treated with high flow Extracorporeal Membrane Oxygenation(ECMO) for a long time was summarized. The key points of nursing include pneumothorax nursing,safety nursing of multiple hospital transfers under the support of high flow ECMO,nursing of multiple conversion mode and replacement of oxygenator under the support of high flow ECMO,nursing of bleeding and thrombotic complications and convulsions. The patient was treated with ECMO for 44 days. After careful treatment and nursing,the lung transplantation was successfully completed and ECMO was successfully removed. The condition improved and the patient was recovering.

Development and application of management information system for the recognition and prevention of incontinence-associated dermatitis

Objective To develop the management information system of recognition,prevention and control of incontinence-associated dermatitis(IAD) and to explore its application effects in the prevention and nursing management of clinical incontinence dermatitis. Methods The management group of IAD was established,and the management information system of IAD identification and prevention and control was constructed. The man-machine interaction technology was used to develop four modules,including evaluation,grading,decision-making and nursing documents,so as to realize the functions of evaluation and early warning,intelligent interpretation of grading judgment, decision-making guidance and record preservation. It was officially put into clinical application in July 2019. The time-consuming,timely rate,accuracy rate,update rate and incidence rate of IAD were compared before and after the application of the system. Results After the application of the information system,the time to implement risk assessment of patients by nurses was shortened from(2.88±0.38) min to(1.44±0.45) min;the timely rate of risk assessment of IAD increased from 52.5% to 82.5%;the accuracy rate of assessment increased from 57.5% to 85.0%,and the update rate of risk assessment of IAD increased from 17.5% to 77.5%(P<0.05). The incidence of IAD decreased from 16.0% to 11.8%(P>0.05). Conclusion The management information system of recognition,prevention and control of IAD can provide accurate and objective decision-making bases for nurses,shorten the time for risk assessment of incontinence-associated dermatitis,improve the timely rate and accuracy of assessment,the dynamic updating rate of risk assessment and grading assessment,and help to improve the quality of skin care for incontinence-associated dermatitis.

Study on the status and influencing factors of health-promoting lifestyle in caregivers of patients with neuropsychiatric systemic lupus erythematosus

Objective To analyze the status and influencing factors of health-promoting lifestyle in caregivers of patients with neuropsychiatric systemic lupus erythematosus.Methods A cross-sectional study was performed on 80 caregivers of patients with neuropsychiatric systemic lupus erythematosus between January,2015,and January,2017,in Peking Union Medical College Hospital. A self-administered general information questionnaire,and Health Promotion Lifestyle Profile Ⅱ(HPLP-Ⅱ) questionnaire was completed,and influencing factors were analyzed.Results The mean age of the participants was(46.42±12.15) years. The mean lifestyle score was (116.82± 32.89),with the subscale nutrition showing the highest score(25.35±5.69) and the subscale stress management showing the lowest score(11.35±5.96). Linear regression showed that caregivers with length of caring <1 year,caregivers without chronic diseases,patients with high rate of occurrence of neuropsychiatric symptoms,and caregivers’ self-perceived burden were independent risk factors for health-promoting lifestyle in caregivers of patients with neuropsychiatric systemic lupus erythematosus.Conclusion Caregivers of patients with neuropsychiatric systemic lupus erythematosus had a moderate health-promoting lifestyle. Healthcare providers should help caregivers with short length of caring,caregivers without chronic diseases,patients with high rate of occurrence of neuropsychiatric symptoms,and caregivers’ self-perceived burden,to improve health-promoting lifestyle through health education,skill training and extended care.
